Sure they do. You and your parent or legal guardian bring photo ID a certified copy of your birth certificate and your social security card, sign a release form, pay the piercing fee and you get it done. I've tattooed and pierced in Dayton since 1980-2010 and those are the forms that I had to fill out for the health departmentt. Go see Glen Scott. Tell 'em That Old Dude sent ya. ;)
